Covering letter on the company’s letter head ( if employed) stating name, passport number, designation, the purpose of travel and the mobile number from the applicant.
Original hotel confirmation voucher along with the payment confirmation receipt.
Registration certificate copy of the hotel / tour operator in Ukraine.
Original overseas travel insurance with the sign and stamp pf the insurance company.
Confirmed return air ticket.
Original latest personal bank statement dually signed and stamped by the bank of last 6 months.
Personal Income tax return of the applicant for the last 3 years.
