5. If self employed

b) Company bank statement for last 06 month with bank seal and sign.
c) ITR last 03 years
6. Income tax papers for the last 03 years
7. Updated bank statement for the last 06 month with bank seal and sign.
8. Valid passport + old passports if any
9. If sponsored by any relative/ Friend then below mentioned documents are also has to carried at the time of Interview.
a) Letter of invitation cum sponsorship from USA addressed to the visa officer, American Consulate, Mumbai. Stating name of the applicant, relationship, nationality, passport no. purpose of visit, period of stay with dates and also to be mentioned that all the expenses including medical boarding and lodging will be borne by the sponsors.
b) Sponsors Passport copy along with visa copy.
